Here they are:
1. Remove the forms and the Rules of the Secretariat Informagiovani (Via Naumachia, 45) - Monday to Friday (17 hours, 30 to 19.30) and the Office of Social Affairs of the City (Piazza Barbacani, 2) - Monday al venerdì (dalle 9,00 alle 13,00) e martedì e giovedì anche dalle 16,30 alle 18,30;
2. Riempire in ogni parte i moduli:
a. Modulo di Iscrizione
b. Scheda di Iscrizione
c. Autorizzazione
d. Dichiarazione
3. Consegnare la documentazione di cui alle lettere a., b., c. e d. di cui sopra unitamente al Certificato ISEE (redditi 2009) presso la Segreteria dell’Informagiovani che provvederà a alla consegna presso l’Ufficio Protocollo entro le ore 19.30 di lunedì 5 Luglio 2010 (scadenza improrogabile!).
4. Per gli ammessi in graduatoria - Consegnare presso l’ufficio Affari Sociali del Comune (Piazza Barbican, 2) -
a. Certificate of good health issued by the family doctor and / or approval authority (at least two days prior to departure of children);
b. A copy of the receipt of payment of registration fee (payment to be made on July 14, 2010 from 16.30 to 18.30 at the office of City Treasurer) whose payment thresholds are listed below: ISEE
from zero to ISEE Euro 7000.00 7001.00 40.00
by ISEE ISEE to over Euro 70.00
that copy must be sent by July 16th 2010 c.
Photocopy of the health card (No. SAUB).
Note: The ISEE Certificate may be issued by CAF (CGIL, CISL, UIL)
